CERTIFICATE OF INSURANCE REQUIREMENTS

PLEASE PROVIDE A CERTIFICATE OF INSURANCE WITH THE MINIMUM REQUIREMENTS AS SHOWN BELOW:

1) Comprehensive, General Liability Insurance with the following minimum limits:

General Aggregate Limit : $1,000,000

Products/Completed Operations Aggregate Limit: 1,000,000

Personal & Advertising Injury Limit: 500,000

Each Occurrence Limit: 500,000

Cooreman Real Estate Group, Inc. must be listed as additional insured.

2) Excess/Umbrella Liability as follows:
Per Occurrence Limit: 1,000,000

3)Workmen's compensation and Employer's Liability Insurance. (Note: If no workman's comp. Insurance then independent contractor affidavit of exemption must be supplied.)

The Certificate of Insurance should specify that a minimum of 30 days notification of cancellation will be provided.

THE CERTIFICATE OF INSURANCE MUST BE RECEIVED BY C.R.E.G. PRIOR TO ANY WORK COMMENCING ON C.R.E.G. PROPERTY .
